Fully completed and signed form.
Copy of policy schedule, relevant endorsements, and applicable excess.
Accident report from Police Station (if applicable).
Premium confirmation.
Reason for late notification (Bryte was not notified of the loss occurring within 30 days).
Third party information (if applicable).
Witness details (if applicable).
Repair quotations from Bryte's preferred panel.
Clear enlarged copy of the driver's license.
Copy of the driver's learner license as well as the accompanying passenger's driver's license.
Copy of the driver's professional driving permit (PrDP) (if applicable).
International driving permit (the permit must be translated into English if the permit was issued in any other language).
